Zusammenfassung

Es wird über den Verlauf eines Patienten mit Autoimmunpankreatitis und begleitender -cholangitis berichtet. Über einen Zeitraum von 3 Jahren seit Erstdiagnose kam es zur Remission unter initialer Prednisolontherapie mit 40 mg täglich. Unter Ausschleichen der Medikation kam es zu wiederholten Rezidiven bei Prednisolondosen unter 20 mg täglich, sowie unter Therapieversuchen mit Azathioprin und Mycophenolat-Mofetil. Nach Therapieeinleitung mit dem monoklonalen Antikörper CD20-Antikörper Rituximab konnte Prednisolon vollständig ausgeschlichen und eine anhaltende komplette Remission erzielt werden. Rituximab sollte als Therapieoption bei rezidivierender Autoimmunpankreatitis/-cholangitis in Betracht gezogen werden.

Abstract

We report a patient with autoimmune pancreatitis and cholangitis. During a period of 3 years and despite therapy with steroids and immunmodulatory drugs such as azathioprine and mycophenolate mofetil he suffered from multiple relapse episodes including bile duct stenoses requiring endoscopic interventions. After initiation of therapy with the monoclonal CD20 antibody Rituximab, steroids could be stopped completely and the patient remains in remission. Rituximab should be considered in therapy of relapsing autoimmune pancreatitis and cholangitis.
