Clinicopathologic analysis and p16 expression in surgically treated primary vaginal carcinoma
Background: Primary vaginal carcinomas (VaCa) rare tumors within surgical treatment of gynaecological oncology. Here we describe surgically treated cases within a gynecologic oncology centre.
Methods: The files of the Department of Gynecologic Oncology at the University Hospital of Leipzig were retrospectively evaluated regarding surgically treated primary VaCa between 1999 and 2014. Clinicopathologic and immunohistochemical analysis for p16 was performed.
Results: 19 cases were identified during the study period. All cases were treated by radical surgery and systematic lymphonodectomy. The median age was 62 years with a wide range between 23 and 76 years. The majority of cases (75%) were diagnosed at stage pT1 and pT2. About one third of cases represented lymph node metastases. 37% of the patients showed recurrent disease. 14/19 cases represented with strong p16 immunostaining, suggesting high risk HPV infection and the majority was accompanied by coilocytic high grade vaginal intraepithelial neoplasia (VAIN 3).
Comment: VaCa are rare tumors within surgical gynecologic oncology with a wide age range. About one third represent with lymph node involvement at time of diagnosis and one third are affected by recurrent disease. Nearly 75% may pathogenetically caused by high risk HPV-infection
