Aufgrund der eingeschränkten Reserven, der reduzierten Lebenserwartung und der hohen Empfindlichkeit gegen Noxen muss der gesamte Versorgungsplan bei Menschen mit der Diagnose Frailty individuell erstellt und kontinuierlich angepasst werden. Es handelt es sich um eine Patientengruppe, die im Verlauf einen unterschiedlich hohen, tendenziell steigenden palliativen Versorgungsbedarf aufweist. Das Spektrum der Betreuung reicht von der Aufklärung über den Zustand bis hin zur Sterbebegleitung.
