Increasing maternal age at the time of giving birth can lead to a higher rate of complications. The aim of this study was to analyse pregnancy outcomes in women aged 45 or older.

186 pregnancies in the maternal age group ≥45 years were compared to 186 pregnancies of 29 year olds over a period of 10 years.

The rate of assisted reproduction (34% versus 3%) and multiple pregnancies (16% versus 5%) was significantly higher in the study group. The mode of delivery also showed significant differences between the two groups with a higher rate of caesarean sections in the advanced maternal age group (59% versus 29%). There was an increased risk for preterm deliveries (28% versus 11%) and prepartal complications such as preeclampsia, gestational diabetes or premature rupture of membranes.

These finding should be taken into account when planning assisted reproduction or during medical care in pregnancies in women with advanced or high advanced maternal age.
