Zusammenfassung
Die Indikationsstellung zur operativen Therapie ist bei der rheumatoiden Arthritis
anspruchsvoll. Mögliche Operationen müssen in den Gesamtkontext der chronischen Erkrankung
eingebunden werden. Neben einer Abstimmung mit der bestehenden medikamentösen Basistherapie
muss die Krankheitsaktivität und der Funktionsanspruch des Patienten mitbedacht werden.
In dieser Übersichtsarbeit sollen die Indikationsgrenzen zur operativen Therapie an
der unteren Extremität bei rheumatoider Arthritis dargestellt werden. Bei den operativen
Verfahren in der Rheumachirurgie handelt es sich um Synovialektomien, Weichteileingriffe,
Resektions interpositionsarthro - plastiken, endoprothetische Versorgungen und Arthrodesen.
Für jedes Verfahren ergibt sich eine Indikation, aber auch eine Indikationsgrenze,
abhängig von der anatomischen Lokalisation, dem Grad der Gelenkdestruktion und möglichen
Voroperationen.
Summary
The indication for surgery is demanding in rheumatoid arthritis. Possible operations
must be integrated into the overall context of the chronic disease. In addition to
coordination with the existing medication-based basic therapy, the disease activity
and the patients expectation of function, must be considered. In this review article,
the indication limits for surgery of the lower extremity in rheumatoid arthritis should
be displayed. The surgical procedures that are used in rheumatoid arthritis are synovectomy,
soft tissue procedures, resection-interpositionarthroplasties, joint replacement and
arthrodesis. For each procedure, an indication is given, but also an indication limit,
depending on the anatomical location, the degree of joint destruction and possible
previous surgery.
