Zusammenfassung
Ziel war die retrospektive Analyse der Wertigkeit von Ganzkörper-Leukozytenszintigraphien
bei Verdacht auf einen lokalen Infekt einer Hüftgelenksendoprothese. In 34 Fällen
wurde die Szintigraphie mit 99m-Tc-HMPAO markierten autologen Leukozyten nicht nur
lokal im Bereich der suspizierten Endoprothese durchgeführt, sondern auf eine Ganzkörperuntersuchung
erweitert. Bei insgesamt 28 von 34 Patienten konnte ein pathologischer Befund im Bereich
des Implantats erhoben werden (82,4%). Bei sechs dieser Patienten wurden zusätzlich
zur Infektion im Bereich der Endoprothese im Rahmen der Ganzkörper-Szintigraphie weitere
Entzündungsherde detektiert. Dabei hatten fünf von sechs Patienten je einen weiteren
Entzündungsherd (Pneumonisches Infiltrat der Lunge, Sigmadivertikulitis, drei Mal
Sinusitis ethmoidalis) und einer von sechs Patienten drei weitere Entzündungsherde
(Pneumonisches Infiltrat der Lunge und Dünndarmschlingenabszess und im Colon descendens).
Bei der Leukozyten-Szintigraphie ist auch bei Verdacht auf einen lokalen Infekt die
zusätzliche Ganzkörperuntersuchung indiziert. In 21,4% der Pa-tienten mit pathologischen
Befunden wurden mit der Ganzkörperuntersuchung zusätzliche Entzündungsherde gefunden.
Summary
The aim was the retrospective analysis of the value of a whole body leucocyte scintigraphy
in case of a suspected local infection of the endoprothesis of the hip joint. In 34
patients with a suspected local infection of the hip joint we made a 99m-Tc-HMPAO
labelled leucocytes scintigraphy with local scans of the hip joint and whole body
scans afterwards. In 28 out of 34 patients there was a pathological result around
the endoprothesis (82,4%). In 6 out of the 28 patients there were additional pathological
results shown in the whole body scintigraphy in other localizations. Five out of the
six patients had one additional localized inflammation (pneumonia of the lungs, diverticulitis
of the sigma, three sinusitis ethmoidalis). One out of the six patients had three
additional localized inflammations (pneumonia of the lungs and abscess of the ilium
and in the colon descendens). Even in case of a suspected local inflammation whole
body leucocyte scintigraphy should be made. In 21,4% of our patients additional pathologic
results could be detected.
