Zusammenfassung
Im Spannungsfeld zwischen optimaler therapeutischer Versorgung des RA-Patienten und
ökonomischer Begrenzung der wünschenswerten Therapie sind weltweit Therapie-Empfehlungen
und Guidelines entwickelt worden, die den Zugang zum Einsatz der hocheffektiven, jedoch
kostenträchtigen TNF-alpha-Inhibitoren regeln. Dabei findet sich ein breites Spektrum
regulatorischer Szenarien von nur einer erforderlichen DMARD-Vortherapie und moderater
Krankheitsaktivität (DAS28: 3,2) in Spanien als am wenigsten restriktiver gegenüber
multiplen vorgegebenen DMARD-Vortherapien und einer ganzen Liste zusätzlich notwendiger
Krankheitsmerkmale in Australien als strengster Variante. Die deutschen Empfehlungen
(zwei DMARD-Vortherapien über insgesamt sechs Monate, kein vorgegebener DAS28-Grenzwert,
mögliche frühere Verschreibung in speziellen Fällen) gliedern sich im internationalen
Vergleich als eher mo-derat und gut handhabbar ein. Dennoch werden in Deutschland
nach wie vor weniger als fünf Prozent aller RA-Patienten mit TNF-Inhibitoren behandelt.
Summary
Between the need for optimal control of disease activity in RA patients and a high
percentage of patients reaching this goal with TNF inhibitor treatment and limits
of high costs of TNF inhibitors on the other hand controversial guidelines and recommendations
for prescribing these biologicals have been developed in many countries. A broad spectrum
of different scenarios includes only one DMARD trial for 3 months before and moderate
disease activity (DAS28 > 3.2) in Spain at one end and three different DMARD courses
and a list of several features of high disease activity necessary in Australia at
the other end. German recommendations with two DMARD courses for altogether six months,
but earlier access to anti-TNF treatment in certain cases, ensure a fair access for
German RA patients in comparison to most of the other countries. Nevertheless currently
less than 5 % of RA patients in Ger-many are treated with TNF inhibitors.
