Zusammenfassung

Hintergrund: Die endovenösen Verfahren bei der Behandlung der Beinvarikose haben in den letzten Jahren an Bedeutung zugenom-men. Radiofrequenz- und Laserbehandlungen finden nun bereits eine breite Anwendung, wobei letztere später eingeführt wurden, weshalb nur wenige Langzeitergebnisse existieren.

Methoden: Wir haben mit der endovenösen Lasertherapie im Januar 2004 begonnen und bis Juli 2010 insgesamt 1765 Extremitäten be-handelt. Unsere Patienten wurden regelmäßig zu Kontrollvisiten eingeladen, sodass wir jetzt in der Lage sind, 5-Jahresergebnisse zu präsentieren. Bewertet wurde der klinische und der FKDS-Befund in Hinblick auf Rekanalisati-on mit oder ohne Reflux, sowie die Länge der rekanalisierten Vene und des eventuellen Refluxes. Verwendet wurde ein 980nm Laser mit linearer Sonde.

Ergebnisse: Die VSM zeigte beim 980 nm-Laser bessere Ergebnisse als die VSP.

Ab 2008 verwendeten wir einen 1470 nm-Diodenlaser mit radialer Sonde. Die Ergebnisse scheinen deutlich besser zu sein, auch wenn wir, verständlicherweise, noch keine Langzeitergebnisse mit der neuen Methode vorweisen können. Da aber die Rekanalisationsrate bei der Behandlung mit dem 980 nm-Laser und li-nearer Sonde in den ersten 1 bis 2 Jahre auftrat, kann bereits ein Vergleich erstellt werden.

Summary

Background: The endovenous methods for the treatment of the varicose vein of the leg have became important in the last years. Radiofrequency and laser therapies have already found a large application, although laser therapies were introduced later. Therefore only a few long-term-results have been published to date.

Methods: We started the method in January 2004. Until August 2010 we treated 1765 extremities. Our patients were invited to control-visits regularly, so we were able to elaborate a follow-up of more than 5 years. The clinical and FKDS-results were valued with regard to the recanalization with and without reflux as well as the length of the recanalized vein and the possible reflux.

Results: 980 nm-laser treatment gave satisfactory results regarding the VSM and poor results regarding the VSP. Today we use a 1470 nm-diode laser instead of a 980 nm, as well as radial laser fibre instead of the linear fibre.

The results seems to be clearly better even though we can not yet show long-term-results with the new method. But still a comparison of the results is possible, because the recanalization rate for the treatment with the 980 nm-laser occurred in the first two years after treatment.
