Summary
A total of 236 TSH stimulation tests were performed using 10 units of TSH intramuscularly.
4 hour uptake of 132 I, serum thyroxine [T4 (D)], T3 binding coefficient [RT3 U ratio] and free thyroxine index [T4 -RT3 index] were estimated before and 18 hours after TSH.
In 88 tests classified by radioiodine uptake change as positive serum thyroxine increased
significantly in only 63.7%, T4 -RT3 index in 61.5%.
Out of 108 tests classified as negative by radioiodine uptake, serum thyroxine increased
significantly in 14.8% and T4 -RT3 index in 13.0%.
The RT3 U ratio procedure used was the least reliable parameter in testing thyroid response
to TSH stimulation, T4 RT3 index was no better than T4 (D) alone.
The change in serum thyroxine levels in response to TSH stimulation is in our opinion
unreliable for the evaluation of TSH tests, and correlates poorly with changes in
radioiodine uptake (r = 0.37), for reasons which are discussed.
Un total de 236 réactions de stimulation par la TSH fut effectué en employant par
voie intramusculaire 10 unités de TSH. La fixation de 132 I après 4 heures, la sérum-thyroxine T4 (D), le coefficient ligand T3 [rapport RT3 U] et l’indice de la thyroxine libre [indice T4 -RT3 ] furent estimés avant et 18 heures après l’administration de TSH.
Dans 88 réactions classifiées comme positives par changement de fixation du radioiode,
la sérum-thyroxine augmenta d’une manière significative dans 63,7% des cas seulement,
l’indice T4 -RT3 dans 61,5%.
Dans 108 réactions classifiées comme négatives par fixation radioiode, la sérum-thyroxine
augmenta d’une manière significative dans 14,8% des cas et l’indice T4 -RT3 en 13,0%.
Le procédé de rapport RT3 U employé était la paramètre le moins sûr dans le test de résponse thyroiodienne à
la stimulation par la TSH, l’indice T4 -RT3 n’était pas meilleur que T4 (D) seul.
Le changement de niveaux de la sérum-thyroxine en résponse à la stimulation par la
TSH est, à notre avis, le critère sur lequel on peut le moins compter pour l’évaluation
des réactions TSH et ne correspond que très pauvrement aux changements de fixation
du radioiode (r = 0,37), pour des raisons qui sont discutées.
Es wurden insgesamt 236 TSH-Stimulationsteste unter Verwendung von 10 Einheiten TSH
intramuskulär durchgeführt. Die 4-Stunden-Aufnahme von 132 J, Serumthyroxin [T4 (D)], T3 Bindungskoeffizient [RT3 U-Verhältnis] und freier Thyroxinindex [T4 -RT3 Index] wurden vor und 18 Stunden nach TSH bestimmt.
In 88 Versuchen, die durch eine veränderte Radiojodaufnahme als positiv klassifiziert
wurden, ergab sich eine eindeutige Zunahme des Serumthyroxin nur bei 63,7% und des
T4 -RT3 Index bei 61,5%.
In 108 Versuchen, die nach der Radiojodaufnahme als negativ klassifiziert wurden,
ergab sich eine eindeutige Zunahme des Serumthyroxin bei 14,8% und des T4 -RT3 Index bei 13,0%.
Das RT3 U-Verhältnis-Verfahren war der am wenigsten zuverlässige Parameter zur Prüfung der
Reaktion der Schilddrüse auf TSH-Stimulation, T4 -RT3 Index war nicht besser als T4 (D) allein.
Die Veränderung des Serumthyroxinwerts als Reaktion auf die TSH-Stimulation ist unserer
Ansicht nach für die Auswertung der TSH-Tests unbrauchbar und korreliert nur geringfügig
mit Veränderungen der Radiojodaufnahme (r = 0,37); die Ursachen hierfür werden diskutiert.
