Die Behandlung von Lippen-Kiefer-Gaumen-Spalten

Zusammenfassung

Lippen-Kiefer-Gaumen-Spalten gehören zu den häufigsten Fehlbildungen des menschli-chen Körpers. Sie nehmen die zweite Stelle nach dem Klumpfuß ein.

Die Behandlung erfolgt am besten interdisziplinär in einem Spaltzentrum in engster Zusammenarbeit zwischen Mund-, Kiefer- und Gesichtschirurgie, Kieferorthopädie, Hals-Nasen-Ohren-Heilkunde (Phoniatrie/Pädaudiologie), allgemeiner Zahnheilkunde und Pädiatrie. Üblicherweise werden der Lippen-NasenKomplex mit etwa 4–6 Monaten und der Gaumenkomplex mit etwa einem Jahr opera-tiv verschlossen. Dabei muss Wert auf die anatomisch korrekte Wiederherstellung der Muskelringe gelegt werden, um langfristig funktionell und ästhetisch gute Resultate zu erzielen. Durch die ständige Überprüfung der Hörfunktion und die Förderung des Laut- und Spracherwerbs können die Kinder in aller Regel normal eingeschult werden.

Der Abschluss der Gesamtbehandlung fällt mit dem Abschluss des Wachstums (bei Jungen oft erst mit dem 21. Lebensjahr) zusammen.

Clefts of lip, alveoulus and palate belong to the most common malformations second only to the clubfoot. The comprehensive treatment should be preferably carried out in an interdisciplinary cleft center where maxillofacial surgeons, orthodontists, ENT surgeons (especially pedaudiologists), dentists and pediatricians work closely together. Usually the lip nose complex will be closed at the age of 4–6 months and the hard and soft palate shortly before the first birthday. It is of utmost importance to reconstruct all muscle rings anatomically correct in order to achieve functional and aesthetic good results in the long run. Hearing and speaking abilities have to be checked regularly by the ENT specialist and a speech pathologist. Normally cleft patients can be sent to school just as all other children. In the vast majority of all cases the regular follow-up may be stopped after the end of treatment with the cessations of growth (age 19–21).
