Incidence of permanent iatrogenic hypoparathyroidism following radical thyroid surgery

Introduction:

Iatrogenic injury to parathyroid glands is unfortunate side effect to some radical operations involving thyroid glad, weather for reasons of primary thyroid disease or infiltrative disease warranting total thyroidectomy. Due to anatomy of parathyroid glands such operations may lead to transient or permanent hypo function of parathyroids because of their injury or removal and is manifested by low PTH. Incidence of low PTH is variable, recovery time is also variable and there is some disconcert in literature as to how to determine state of permanent hypoparathyroidism.

Material and Method:

This retrospective study shall encompass all patients who had undergone radical thyroid operations, with parathyroid preservation by reimplantation in Sarajevo clinical University center in past 10 years. We shall compare PTH levels prior to operation and compare them to immediate postoperative and recovery levels.

Results:

By following postoperative levels of PTH from remaining or reimplanted paratyroids we shall determine rate of recovery of parathyroid function and determine need for partial or full medical supplementation.

Conclusion:

Full recovery of paratyroid function means levels of PTH of at least 10pg/mL, no hypocalcemic symptoms and no need for calcium or calcitrol supplements. It is to be expected within 6 – 12 months.
