Is ultrasound-guided core needle biopsy suitable for the diagnosis of a cervical manifestation of a lymphoma?

Introduction:

The aim of our study was to evaluate the role of core needle biopsy for the diagnosis of cervical manifestation of lymphomas.

Method:

The present study was based on a retrospective analysis of the medical records of all patients who underwent ultrasound-guided core needle biopsy between 2006 and 2017 on suspicion of cervical manifestation of lymphoma.

Results:

37 patients diagnosed with lymphoma later on were analyzed retrospectively. In 28 patients (75.6%) definitive lymphoma diagnosis could be made by core needle biopsy. In the remaining 9 cases the definitive diagnosis was confirmed by a lymph node extirpation following the core needle biopsy. In 4 out of these 9 cases, core needle biopsy could set suspicion for a lymphoma, but the tissue sample was not sufficient for further immunohistochemistry. In one case, core needle biopsy incorrectly set the suspicion for lymphoma. Thus, our analysis yielded a sensitivity of 86% with a positive predictive value of 97%. Except for one case, all procedures were performed under local anesthesia. In one case, an aneurysm spurium of the common carotid artery occurred, which had to be managed by means of vascular surgery.

Conclusion:

The ultrasound-guided core needle biopsy is a useful tool for the reliable diagnosis of a cervical manifestation of a lymphoma with a low morbidity rate. In limited cases, a diagnostic extirpation or a wedge excision of the suspicious lymph node may be necessary for confirmation of the diagnosis und further immunohistochemistry.
