Extrapulmonary manifestation of sarcoidosis in the paranasal sinus

Introduction:

We report the unusual case of a 31-year-old male patient with nasal congestion for two years, moreover he reported hyposmia.

Methods:

On anterior rhinoscopy, the nasal mucosa presented marked hypertrophy, purplish discoloration and granulomatous appearance of the medial turbinate on the left side. A CT scan revealed hypodense soft tissue mass in the left nasal cavity extending upto the left maxillary and ethmoidal sinus.

Results:

The diagnosis of sinonasal sarcoidosis was made based on histopathologic examination of biopsy specimens.

Conclusion:

Sarcoidosis is a multisystem granulomatous disorder and up to 30% of patients present with extrapulmonary sarcoidosis, but rarely in the paranasal sinus (1%). As no further organ affection could be detected, treatment was started with regular nasal irritation with saline solution and installation of intranasal glucocorticoids. Furthermore systemic glucocorticoids and other immunosuppressive agents maybe needed for severe disease later.
