Zusammenfassung

Aufgrund der Daten vorliegender Studien scheint eine orale Antikoagulation niedriger Dosierung in der Prävention des Schlaganfalls bei Vorhofflimmern die besten Resultate zu bringen: Bei wirkungsvoller Reduktion der zerebralen Insulte kommt es dabei praktisch zu keiner Zunahme zerebraler Blutungen im Vergleich zur Plazebogruppe. Parallel zum Quick-Wert ist offensichtlich die Reduktion des Plasmaspiegels des Prothrombinfragments F1+2 direkt abhängig von der Intensität der oralen Antikoagulation und daher zur individuellen Kontrolle des erreichten antikoagulatorischen Effekts geeignet. Die anzustrebende optimale Antikoagulation sollte Quick-Werte um 35% bzw. INR-Werte um 2,0 induzieren.
