Hämodilution beim akuten ischämischen Hirninfarkt

Zusammenfassung

Die Hämodilution ist eine der vier heute verfolgten Therapiestrategien beim akuten ischämischen Hirninfarkt. Sie kommt zum Einsatz als spezifische Monotherapie oder in Kombination mit dem Neuroprotektivum Piracetam beim Wegfall der Indikationen zur lokalen oder systemischen Lyse, zur Frühvollheparinisierung mit nachfolgender oraler Antikoagulation, zur frühen gefäßchirurgischen Intervention und frühen PTCA an den hirnversorgenden Arterien. Ihre Anwendung geht von einer Verbesserung der Mikrozirkulation im Infarkt- und Infarktrandgebiet über hämorheologische und hämodynamische Mechanismen als Wirkprinzip aus. Behandlungsverfahren der Wahl ist die der individuellen Belastbarkeit angepaßte hypervolämische Hämodilution mit 10% HES 200/0,5 oder 10% Dextran 40. Mit beiden Plasmaexpandern sind in kontrollierten Langzeitstudien günstige Wirkungen auf wichtige hämorheologische und hämodynamische Parameter erbracht worden. Beide Kolloidlösungen verbessern signifikant die zerebrale Durchblutung mit überproportionaler Perfusionssteigerung im Infarktrandgebiet. Die bisherigen kontrollierten klinischen Studien haben widersprüchliche Ergebnisse geliefert. Während mit einer ausschließlich normovolämischen Hämodilution positive Therapieeffekte nicht zu erzielen sind und bei therapieinduzierter Hypovolämie Befundverschlechterungen resultieren, haben die hypervolämischen Hämodilutionsstudien ausnahmslos positive Befunde erbracht, welche die klinische Anwendung beim akuten ischämischen Insult rechtfertigen. Ein Konsens über den Wert der hypervolämischen Hämodilution konnte jedoch bis heute nicht erreicht werden. Grund dafür sind offene Fragen zur Patientenselektion, Dosierung, Behandlungsdauer und zum Zeitfenster, die nur in groß angelegten, klinisch kontrollierten Studien zu beantworten sind.
