Mastoidal manifestation of histiocytosis in a young woman

Introduction:

Histiocytosis is a rare systemic disease. It is defined as an inflammatory myeloid neoplasm. Isolated pulmonary manifestations are most common in adults.

We present a young woman with an extended manifestation in the mastoid.

Case

The Patient presented initially otalgia and otorrhea. Examination showed an inflammation of the outer ear canal. The patient was diagnosed with pulmonary histiocytosis two years ago, but no therapy was required at that time. A diabetes insipidus was also known.

A few days later she returned with vertigo. The CT scan revealed an extendend osteolytic process of the mastoid. A mastoidectomy was performed. Histology showed a manifestation of histiocytosis.

Therapy

Chemotherapy with cytarabine was started. So far, the patient showed no complications.

Conclusion:

Histiocytosis is a rare disease. Pulmonary and osseous manifestations are most common.

In our case, pulmonary manifestations in combination with diabetes insipidus as a first sign of a manifestation of the central nervous system led early to the diagnosis of a manifestation of the mastoid.
