Simultaneous implantation of an active bone conduction hearing implant (Bonebridge®) and a bone anchored epithesis

Introduction:

The combination of a hearing implant and an epithesis in aural atresia offers satisfactory auditory rehabilitation and cosmetic reconstruction. The operative procedure is often performed in sequential surgical interventions. The aim of the study was to gain experience with a simultaneous approach, in which the hearing implant and the bone anchored epithesis were implanted simultaneously.

Method:

Two patients with aural atresia (Type III Weerda) received a Bonebridge® and a bone anchored epithesis in one operation. Previously, the indication for the use of an active transcutaneous bone conduction hearing implant was determined using the AMEI – score (Frenzel et al. 2013). The application of the abutment was carried out on an outpatient basis after the activation of the hearing implant.

Results:

Both patients scored 4 points each in the AMEI-score, which displays a low chance for the sucessful implantation of a Vibrant Soundbridge®. The implantation of the Bonebridge® and the bone anchored epithesis was then performed without complications. After a regular post-operative course the first activation and the application of the abutment (magnetic inserts) was performed 4 weeks later. The epithesis was reshaped to the healthy ear. Both patients where highly satisfied with the cosmetic and the auditory outcome.

Conclusion:

The rehabilitation with a hearing implant and an epithesis represents a good alternative for patients with aural atresia. The AMEI score was a useful tool for the indication. Using the simultaneous approach the surgical effort and the time of rehabilitation could be further reduced.
