Surgical treatment of thyroid tumor in China

The incidence of thyroid tumor is increasing in China. For benign lesions, we limit the surgical indications to those are visible in neck or have compressed symptoms. Recently we resected some cases of post-sternal nodular goiter by low cervical incision. The endoscopic surgery and cosmetic surgery with minimal incision are applied for the early stage carcinoma. For the tumors invading the outer membrane or with small depth of tracheal cartilage, we incise the tumors from tracheas but with the integrity of airway. A well-placed drainage may prevent postoperative dyspnea. However, when the tumors invading the whole layer of cartilage, or the mucous, we resect the corresponding part of tracheas. Even after resected up to 6 rings of tracheal cartilage, the tracheas may be sutured by the end-to-end anastomosis. In some cases, the tracheotomy may be prevented. When the tracheal defect is too large to close, we may leave a tracheostomy and seal the tracheas by cervical skin flap in second stage of surgery. The pectoris major flap and the sternocleidomastoid periosteal flap may be used for the limited lateral tracheal defects. The radioiodine and neck dissection are also supplement for thyroidectomy. In recent year we performed approximately 2000 operations for thyroid disease annually, including the late stage cases approximately 10 percent. The 5-year survival of DTC was 94 %, whereas almost 100 % for the stage I & II cases. The 10-year survival of DTC was 89 %, while the 5-year survival of Stage IV DTC was only 50 %. The prognosis of medullary thyroid carcinoma was even worse than that of DTC.
