Rare differential diagnosis of an extensive lesion of the base of the tongue: Chronic lymphocytic leukemia

Introduction Chronic lymphocytic leukemia (CLL) is the most common form of malignant neoplasm of the lymphatic system in Western nations. It is responsible for 25 % of all leukemias and occurs especially in old age. The disease is characterized by a changeable clinical appearance and a widely varying prognosis. Lymph node swelling, spleen and liver enlargement and B symptoms may occur.

Method A 65-year-old patient presented with dysphagia, evening sore throat and muffled Voice for about 4 weeks without B symptoms. The clinical examination revealed an extensive oropharyngeal mass with a smooth mucosal lining, starting from the right base of the tongue with almost complete displacement of the larynx. With suspicion of a retention cyst, the indication for immediate surgical repair and protection of the airway was made Results: The anesthesia was initiated with tracheotomy readiness. Intraoperatively, a massive lesion was found from the base of the tongue with a smooth mucous membrane with displacement of the epiglottis, which was almost completely resected. Histological examination revealed a chronic lymphocytic leukemia. The patient was transferred to an oncological center for further therapy.

Discussion The tongue base as part of the Waldeyer pharyngeal ring contains lymphatic tissue, so that chronic lymphocytic leukemia as a differential diagnosis for tumors in this region must be considered, even if it is a rare finding
