Introduction The surgical microscope has been the standard technique in ear surgery for many decades. However there are limitations in using the microscope in certain situations. Only partial visualization of the surgical field is one of the limitations for example. An open surgical approach is required in the majority of cases. The transcanal endoscopic approach provides a minimally invasive method with improved visualization of the surgical field.

Method The advantages and disadvantages of endoscopic ear surgery are presented in comparison with the microscopic technique.

Results The endoscopic technique facilitates a panoramic view of the the entire tympanic cavity as well as excellent detailled views of specific regions. Furthermore regions, which are invisible with the microscope can be visualized. There is high flexibility regarding view, scope and magnification of the situs. Tissue trauma is minimized. As there are no sutures and no scars, patient comfort is high. One handed operation technique and narrow space in the external ear canal represent impairments for the surgeon in the beginning.

Conclusion: The transcanal endoscopic technique represents a reasonable methodical addition that can be used as an alternative or in addition to the use oft he surgical microscope.
