Changes of oscillation pattern changes due to loudness in patients with vocal fold mass lesions

Introduction Vocal fold mass lesions might affect vocal fold oscillation patterns and therefore voice production. It has been observed before that irregularity values were lower for greater loudness. However, how much oscillation patterns change with gradual changes of loudness has not been understood, in detail.

Material and Methods Eight patients with vocal fold mass lesions were asked to perform a glide from minimum to maxium loudness on the vowel /i/, o on125Hz for male or 250Hz for female voices. During phonation the subjects were recorded with transnasal high speed videoendoscopy (HSV, 20.000fps), electroglottography (EGG), and audio signals.

Results Greatest vocal fold irregularities were observed not at maximum SPL but between minimum and maximum SPL. There was a great disagreement of HSV and EGG derived measures with regard to open quotient and measures of irregularity.

Conclusions The amount of vocal fold irregularities are changing with changing loudness. This should be considered in evaluation of oscillation patterns. The measures from EGG and GAW show much stronger differences in patients with vocal fold mass lesions than in physiologic data. Therefore especially EGG based data has to be interpreted with caution.
