Myxoma of the nasal columella

Introduction We would like to present a rare rhinological case. A myxoma which developed in the area of the columella.

Clinical history & process The 59 yrs old patient presented himself in our clinic with persistent swelling of columella for 10 days which was painless and increasing. Initial assessment showed an elastic mass together with a perforated nasal septum which was known by status after septoplasty in 2013 alio loco. In his history diseases like systemic illnesses, allergies and medications were negative. MRI scan of cranium/face showed a cystoid formation in area of his tip of the nose or of the columella with a bit of contrast material- enhancement, which most likely looked like a retention cyst. Next step was punction of the mass but there was no secretion. We treated the patient with oral antibiotics and local therapy. Since there was no improvement despite therapy we decided to remove the mass throuh an operation. The histological result showed a benign myxomatous neoplasm or rather a facial myxoma of the columella.

Discussion The myxoma is a benign tumor which can grow invasively and destructive. It is known as one of the most common cardiac primary tumors which grows mostly in the left atrium. It can also come up with syndromes like ' Carney Syndrome ' for example. The ENT area espescially the nose and paranasal sinuses are an uncommon part for this type of mass. Treatment is an operative resection of the tumor.

Conclusion As a differential diagnosis in case of growths of the columella is to be thought also of the myxoma although the location in the area of the nose is rather seldom. In such cases the diagnosis can only be confirmed with histopathological examinations.
