Long-term results of shortening osteotomies in total hip replacement in patients with congenital hip dislocation

Objectives analysis of the long-term results of THR in patients with dysplastic coxarthritis Crowe IV

Methods We analyzed the results of THR in 102 patients with dysplastic coxarthritis Crowe IV who were operated between 2011 and 2018. The average age was 41.8±3.9 years. Gender distribution: 83 women and 19 men. All patients were underwent cementless THA. Cup was placed in true acetabulum. The shortening femoral osteotomy was performed in the subtrochanteric zone and stem was placed. The type of shortening femoral osteotomy was chevron. Treatment outcomes were evaluated by clinical (HHS, OHS, UCLA) and X-ray methods after 3, 6, 12 months and then annually. Average follow-up period was 5.2±2.3 years.

Results and Conclusion Results. According to HHS, average postoperative result was 86.1±3.3. Maximal increasing of results were noted in postoperative period between 3 and 6 months. Difference of lower limb length was 0,6±0,3 cm. Range of motion in the hip joint was full. Physical activity was evaluated by UCLA. The average score - 7.6±1.3. According to OHS, average postoperative result was 41.1±1.4. Union of osteotomy was evaluated by X-ray and CT (for patients who had doubtful signs of union). Full union of the femur occurred after 2.3±0.2 months. Non-unions of the femur were noted in 2 cases (1.9 %). Aseptic femoral loosening occurred in 3 hip (2.9 %).

Conclusions. Chevron femoral osteotomy in patients with congenital hip dislocation is characterized with high functional results and rapid union of the osteotomy zone.
