Vorwort

Die wissenschaftliche Evidenz zur Versorgung von Kindern nach Fremdkörperaspiration und Fremdkörperingestion ist insgesamt gering. Es gibt zu diesem Themenkomplex keine randomisiert-kontrollierten Studien (RCTs), und es ist nicht zu erwarten, dass sich daran künftig etwas ändert. Die vergleichsweise robustesten Daten stammen aus mehr oder minder großen, unkontrollierten Fallserien bzw. einzelnen Fallberichten. Darüber hinaus existieren strukturierte Expertenübersichten sowie 2 nationale, US-amerikanische Praxisleitlinien zur Fremdkörperingestion.

Insgesamt ist die Versorgung nach Fremdkörperaspiration und Fremdkörperingestion primär durch strukturelle, organisatorische und praktisch-klinische Aspekte gekennzeichnet. Diese wurden von den an dieser Leitlinie beteiligten und von ihren jeweiligen Fachgesellschaften legitimierten Experten aus allen themenrelevanten Disziplinen auf der Basis eigener fundierter Expertise und klinischer Erfahrung beschrieben. Darüber hinaus wurde die vorhandene wissenschaftliche Evidenz evaluiert und eingearbeitet. Da zudem die bewährten Vorgehensweisen („best clinical practice“) institutionell z. T. erheblich variieren, wurden die Empfehlungen insgesamt eher weit gefasst. Dabei wurden abgestufte Formulierungen gemäß der AWMF-Empfehlungsgraduierung wie „soll“/“soll nicht“, „sollte“/“sollte nicht“ und „kann erwogen werden“/“kann verzichtet werden“ verwendet.

Die Erstellung dieser S2k-Leitlinie erfolgte gemäß AWMF-Protokoll nach umfassender Evidenzevaluation auf der Basis eines Delphi-basierten Konsensusprozesses einer interdisziplinären Expertengruppe, mit abschließender Konsentierung durch die Präsidien bzw. Vorstände der beteiligten Fachgesellschaften. Die Expertengruppe bestand aus erfahrenen, von ihren jeweiligen Fachgesellschaften legitimierten Kinderanästhesisten, Kinderintensivmedizinern, pädiatrischen Pneumologen, pädiatrischen Gastroenterologen, Hals-Nasen-Ohren-Ärzten und Kinderchirurgen.
