Zusammenfassung

Hintergrund Die endovaskulären Techniken haben die Therapie von Erkrankungen der abdominellen und thorakoabdominellen Aorta revolutioniert. Für die Versorgung im Bereich der infrarenalen Aorta ist die endovaskuläre Therapie mittlerweile ein Standardverfahren für Elektivfälle und im Notfall. Für komplexe, d. h. die renoviszeralen Gefäße einbeziehende, abdominelle und thorakoabdominelle Aortenpathologien bestehen sowohl offen als auch endovaskulär große technische Herausforderungen. Aufgrund der hohen Mortalität und Morbidität der offenen Chirurgie, insbesondere im Notfall, haben sich auch hier verschiedene endovaskuläre Techniken, insbesondere für Mittel- bis Hochrisikopatienten, zur Versorgung entwickelt.

Ergebnisse Zu den endovaskulären Therapieoptionen zählen grundsätzlich die fenestrierten und gebranchten Stentprothesen, die entsprechend der Anatomie runde Aussparungen („Fenestrierung“) oder Abzweigungen („Branches“) für die renoviszeralen Gefäße haben, sowie die Chimney-Graft-Technik. Im Elektivfall können patientenindividuelle fenestrierte und gebranchte Prothesen („custom-made“) bestellt werden. Planung, Produktion und Lieferung erfordern jedoch eine Wartezeit von bis zu 12 Wochen. Im dringlichen Fall steht aktuell nur eine 4-fach gebranchte und patientenunspezifische, d. h. „off-the-shelf“ („von der Stange“) Stentprothese zur Verfügung, die für nur schätzungsweise 60% der Patientenanatomien mit komplexem abdominellem und thorakoabdominellem Aortenaneurysma passt. Für Patienten ohne passende Anatomie stehen die sog. „surgeon-modified“ oder „vom Chirurgen modifizierten“ Stentprothesen als Alternative zur Verfügung. Hierbei wird eine kommerzielle Prothese direkt präoperativ mit den notwendigen Fenestrierungen modifiziert. Verglichen mit den „off-the-shelf“ Stentprothesen weist die Literatur aktuell für die „surgeon-modified“ Technik ähnliche Ergebnisse bez. Morbidität und Mortalität auf.

Schlussfolgerung Die „surgeon-modified“ Stentprothesen haben ihren Stellenwert bei Hochrisikopatienten mit symptomatischen und gedeckt rupturierten komplexen Aneurysmen. Bis kommerzielle fenestrierte und gebranchte Stentprothesen für alle anatomischen Gegebenheiten als „off-the-shelf“ Prothesen vorhanden sind, stellt die „surgeon-modified“ Technik eine gute Behandlungsoption dar.

Abstract

Background Endovascular techniques have revolutionised the therapy of abdominal and thoracoabdominal aortic disease. For infrarenal abdominal aortic aneurysm, the endovascular aortic repair has become a standard for elective and emergent cases. In complex abdominal or thoracoabdominal aortic pathologies, involving reno-visceral vessels, there are technical challenges for open and endovascular surgery. Due to high mortality and morbidity of open surgery of complex aortic lesions, especially in emergent cases, endovascular techniques have developed as well.

Results Endovascular treatment options for complex aortic pathologies are fenestrated and branched stent grafts and the chimney graft technique. In elective cases, fenestrated and branched stent grafts are ordered as “custom-made” devices but planning, production and delivery takes up to approximately 12 weeks. For urgent cases, there recently only exists one 4-vessel branched “off-the-shelf” stent graft, that fits only about 60% of patientsʼ anatomy in complex abdominal or thoracoabdominal aneurysm cases. As an alternative for these patients, “surgeon-modified” stent grafts are a treatment option. Here, a commercially available stent graft is modified with the needed fenestrations and branches for the visceral vessel prior to the operation. Compared to off-the-shelf stent grafts, the surgeon-modified stent grafts have similar results for mortality and morbidity.

Conclusion As long as off-the-shelf devices for a larger variety of abdominal and thoracoabdominal anatomy are available, surgeon-modified stent grafts are a good treatment alternative for urgent complex abdominal and thoracoabdominal aortic pathologies in high-risk patients.
