Symptomatische Tachyarrhythmie bei einem Herzschrittmacherpatienten in der Notfallmedizin

Zusammenfassung.

In der präklinischen Notfallmedizin können Patienten mit Herzschrittmacher bei Vorliegen einer ernsten kardialen Symptomatik aufgrund von Arrhythmien für den anwesenden Notarzt eine besondere Herausforderung bedeuten. Vorgestellt wird der konkrete Fall eines Patienten mit Angina pectoris bei einer Schrittmachertachyarrhythmie. Es wird ein strukturiertes Vorgehen unter Berücksichtigung der möglichen Ursachen und einer präklinisch sinnvollen Therapie vorgestellt. Insbesondere der vielfach in Lehrbüchern empfohlene Einsatz eines Magneten sowie die Pharmakotherapie werden kritisch diskutiert. Symptomatic tachyarrhythmia in the presence of a cardiac pacemaker is challenging in preclinical emergency situations. This article presents a case report and a diagnostic and therapeutic concept for prehospital treatment of such cases. The usefulness of extracorporeal magnet application which is recommended in the literature and the possibilities of antiarrhythmic therapy will be critically discussed.
