Zusammenfassung
Eine 25-jährige Patientin mit einer seit 3 Jahren bestehenden ungeklärten schweren
Obstipation wurde mit Verdacht auf einen soliden Unterbauchtumor eingewiesen. Es ergab
sich die Diagnose eines primären strumalen Karzinoids des Ovars. Nach Entfernung des
Tumors hatte die Patientin eine normale Darmtätigkeit. Im trabekulären Tumoranteil
gelang der immunhistochemische Nachweis von Peptid YY (PYY). Dies ist ein physiologisches
Peptidhormon, das einen starken inhibitorischen Einfluss auf die Darmmotilität besitzt.
Somit konnte der Beweis erbracht werden, dass eine tumorbedingte Überproduktion von
PYY die Ursache der Obstipation war. Bisher sind nur 6 weitere derartige Fälle beschrieben
worden.
Abstract
A 25-year-old woman with a 3-year history of severe constipation presented with a
solid pelvic mass, which turned out to be a primary strumal carcinoid of the ovary.
Immunohistochemistry showed peptide YY, a physiologic peptide hormone with a strong
inhibitory effect on intestinal motility, in the trabecular portion of the tumor.
Bowel function normalized after extirpation of the tumor, indicating that constipation
was due to increased peptide YY synthesis by the tumor. Only six such cases have been
described previously.
