Zusammenfassung
Eine frühe Defibrillation bei Kreislaufstillstand durch Kammerflimmern stellt die
einzig adäquate Therapie dar. Die Defibrillation ist damit ein wichtiges Glied in
der Rettungskette, wobei es offen bleiben muss, wer die Applikation des Stromstoßes
durchführt. Voraussetzung für die Wirksamkeit ist die Einhaltung verpflichtender Algorithmen,
die in Theorie und Praxis präsent sein müssen. Für die Durchführung einer Frühdefibrillation
durch Ersthelfer muss die Überprüfung der Fähigkeiten und die Durchführung der Maßnahme
unter ärztlicher Verantwortung erfolgen. Dem verantwortlichen Arzt muss ein Weisungsrecht
eingeräumt werden. Sofern die Erstdefibrillation im Rahmen eines freiwilligen Ersthelfersystems
durchgeführt wird, ist sie grundsätzlich optional und nicht Teil eines solchen. Die
Laiendefibrillation im Ersthelfersystem setzt Engagement und Ausdauer zur Aufrechterhaltung
der Befähigung voraus. Ersthelfersysteme sind kein Ersatz für Organisationslücken
im Rettungssystem, sondern stellen nur ein additives Versorgungselement dar.
Early Defibrillation by First-Aid Personnel - Risk or Ouality Drop
Early defibrillation is the only adequate treatment of circulatory arrest due to ventricular
fibrillation. Hence, defibrillation is a significant link in the sequence of first-aid
rescue measures but it remains tobe determined who applies the impulse. The effectivity
depends entirely on the fact whether the theoretically and practically essential algorithms
are complied with. If defibrillation is to be performed by first-aid personnel, their
qualification and the performance must be effected under medical supervision. Initial
defibrillation within the framework of a first-aid team system is principally optional
and not part of the rescue system. Defibrillation by laypeople essentially requires
engagement on the part of the team and endurance in keeping up the qualifications.
First-aid systems are not stopgaps for deficiencies in the rescue system organisation,
they are merely an additional care element.
