Zusammenfassung
Einleitung: Das Nierenzellkarzinom ist die häufigste während einer Schwangerschaft diagnostizierte
renale Neoplasie. Die Wahl des richtigen Operationszeitpunktes ist umstritten.
Kasuistik: Anhand zweier Fallberichte werden klinischer Verlauf, sowie diagnostisches und therapeutisches
Vorgehen beim schwangerschaftsassoziierten Nierenzellkarzinom geschildert.
Schlussfolgerung: Sonographie und Kernspintomographie sind verlässliche und nach heutigem Wissen in
der Schwangerschaft unbedenkliche Verfahren zur primären Diagnostik und präoperativen
Stadieneinteilung. Bei tumorbedingten Komplikationen, psychosozialer Indikation oder
Diagnose im ersten Trimenon sollte die radikale Tumornephrektomie unverzüglich durchgeführt
werden. Im zweiten und dritten Trimenon kann bei unkompliziertem Schwangerschaftsverlauf
mit der Operation bis nach der Entbindung gewartet werden.
Abstract
Introduction: Renal cell carcinoma is the most common renal neoplasm in pregnancy. The best timing
for surgical treatment is a matter of dispute.
Case reports: The clinical course of renal cell carcinoma in pregnancy as well as therapeutic and
diagnostic options are described in two case reports.
Conclusions: Ultrasound and magnetic resonance imaging are reliable methods for primary diagnostics
and preoperative staging without known adverse effects on the fetus. Under certain
circumstances, such as tumor-associated complications, psychological indication or
diagnosis of the tumor during the first trimenon, radical nephrectomy should be carried
out without delay. In the second and third trimenon and in uncomplicated pregnancies,
surgery is best done after delivery.
