Zusammenfassung
Mit der zunehmenden Überalterung der Bevölkerung in Deutschland ist in den letzten
Jahren auch ein stetiger Anstieg proximaler Femurfrakturen zu verzeichnen. In den
letzten Jahren hat sich die Behandlungsstrategie erheblich gewandelt, wodurch deutlich
bessere Ergebnisse erzielt wurden. Dieser Erfolg wurde ermöglicht durch eine fortlaufende
Verbesserung der Operationstechnik, der Implantate sowie der interdisziplinären Zusammenarbeit
sowohl im perioperativen Management als auch in der postoperativen Phase bis hin zu
rehabilitativen Maßnahmen.
Es werden die derzeit aktuellen OP-Verfahren vorgestellt und durch einen einfachen
Algorithmus bei medialen Schenkelhalsfrakturen verdeutlicht.
Aus dem eigenen Krankengut von 1995- 1999 wird über die Ergebnisse von 709 operativ
behandelten Patienten mit proximalen Femurfrakturen berichtet, die 70 Jahre und älter
waren. Die angewandten Operationsverfahren und deren Ergebnisse werden kurz erläutert
und im Vergleich zu den Mitteilungen aus der aktuellen Literatur diskutiert.
Die Krankenhausletalität liegt bei 7,2 %. Revisionspflichtige methodenspezifische
Komplikationen finden sich bei 3 % des Gesamtkollektivs. Revisionspflichtige Infekte
sind bei 2,7 % der Patienten aufgetreten (oberflächliche Wundheilungsstörungen eingeschlossen).
Circa 30 % der Patienten können in das häusliche Umfeld zurückkehren, nahezu 40 %
müssen in Pflegeheime bzw. geriatrische Einrichtungen verlegt werden. Circa 30 % sind
in einer AHB-Klinik weiterbehandelt worden. Der Anteil an Reha-fähigen Patienten liegt
mit 47,5 % bei den Schenkelhalsfrakturen deutlich höher als bei den pertrochantären
Frakturen (16,6 %). Offensichtlich ist die perioperative Morbidität bei den Patienten
mit pertrochantären Frakturen höher als bei den Schenkelhalsfrakturen.
Proximal femur fractures in the elderly patient. Treatment algorithm and social aspects
Summary
With increasing life expectancy in the German population in the past years we see
a steady increase of proximal femur fractures. In recent years the surgical approach
to proximal femur fractures has changed tremendously, resulting in significantly better
outcome than in the past. This success was made possible due to constant improvements
in surgical treatment, surgical implants and in an interdisciplinary co-operation
lasting from perioperative management as well as post-operative care to rehabilitation
services.
The present surgical treatment for femoral neck fractures and peritrochanteric fractures
is presented and the application of these therapies is illustrated with an easy to
use algorithm. Results of 709 patients who had to undergo surgical treatment because
of proximal femur fractures are presented. The patients were treated in the years
1995-1999 and their age was 70 and older. The applied surgical treatment and the results
are elucidated and compared to the recent medical literature.
The average hospital mortalilty was 7.2 %. Methodical complications which required
secondary surgery where seen in approx. 3.0 % of our study group. Wound infections
(including superficial wound healing complications) necessiating secondary surgery
occured 2.7 %. Approximately 30 % of all patients were able to return back home respectively
to their prior status. Almost 40 % of the patients required accomodation in a nursing
facility or a geriatric center. In continuitation of their treatment nearly 30 % of
all patients were transfered to a temporary rehabilitation center. The percentage
of patients capable of undergoing further treatment in a temporary rehab center lies
significantly higher within patients with femoral neck fractures with approx. 47.5
% vs. 16.6 % in patients with pertrochanteric fractures. Apparently, the perioperative
morbidity ist higher in patients with pertrochanteric fractures than in patients with
femoral neck fractures.
Résumé
En raison du vieillissement croissant de la population en Allemagne depuis ces dernières
années, le nombre de fractures proximales du fémur est en augmentation constante.
Durant ces dernières années, la stratégie thérapeutique s'est substantiellement modifiée,
se traduisant par une amélioration notable des résultats. Ces succès ont été rendus
possibles grâce à la constante amélioration des techniques opératoires, des implants
ainsi que du travail interdisciplinaire, du management périopératoire et aussi de
la phase postopératoire jusqu'aux mesures de réhabilitation. Les procédés opératoires
actuels sont présentés et précisés à l'aide d'un algorithme pour les fractures médiales
du col du fémur. Les résultats de 709 patients issus de notre collectif de malades
de 1995 à 1999 opérés pour fractures proximales du fémur âgés de 70 ans et plus sont
rapportés. Les procédés opératoires utilisés et leurs résultats sont brièvement exposés
et discutés en les comparant avec les publications de la littérature actuelle. La
létalité hospitalière se situait à 7,2 %. Des complications liées aux méthodes nécessitant
une révision sont retrouvées dans 3 % du collectif total. 2,7 % des patients ont présenté
une infection nécessitant une révision (y compris les troubles superficiels de la
cicatrisation). Environ 30 % des patients ont pu rentrer chez eux, presque 40 % ont
dû être transportés dans une maison de soins ou une institution gériatrique. Environ
30 % ont poursuivi le traitement en Centre de Réadaptation. Le pourcentage de patients
capables de se réadapter est nettement plus élevé pour les fractures du col (47,5
%) que pour les pertrochantériennes (16,6 %). La morbidité périopératoire des fractures
trochantériennes est manifestement plus élevée que celle des fractures du col.
Resumen
Con el aumento de la esperanza de vida de la población germánica, vemos un aumento
también preciso de las fracturas proximales de fémur. En los últimos años la cirugía
en estas lesiones presentaron un significativo cambio en los resultados con respecto
al pasado. El éxito de los mismos fue hecho posible debido al constante perfeccionamiento
en cirugía ósea, en variaciones de implantes y una cooperación interdisciplinaria
desde el manejo pre, peri y post operatorio de rehabilitación. En resultados sobre
700 pacientes tratados entre 1995-1999 con edades no menores de 70 años se observaron
los siguientes datos:
• Promedio de mortalidad hospitalaria fue 7,2 %
• Complicaciones del método que requirieron revisión secundaria fue aproximadamente
3,0 %
• Infecciones que necesitaron nueva cirugía un 2,7 %. Aproximadamente el 30 % de todos
los pacientes retornaron a su situación previa a la lesión fracturaria con capacidades
completas. En un 40 % debieron necesitar colaboración y nursing facilidades o ingresar
a un centro geriátrico para culminar la rehabilitación. Los pacientes que requirieron
mayor rehabilitación fueron los que presentaban fracturas de cuello femoral con respecto
a las peri trocantéricas fracturas (47,5 % vs 16,6 %). Pero aparentemente la morbilidad
per operatoria es mayor en los pacientes con fracturas per trocantéricas que con respecto
a los de fracturas cervicales.
