Bindungscharakteristika und Behandlungserfolg nach stationärer psychodynamischer Gruppenpsychotherapie

Zusammenfassung

In einer Reanalyse von Daten der „Kieler Gruppenpsychotherapiestudie” wurden die videoaufgezeichneten Indikationsgespräche mit 34 Patient(inn)en mit dem Erwachsenenbindungsprototypenrating analysiert. Drei Subgruppen, die nach den kategorisierten Bindungsmustern (vermeidend, ambivalent, gemischt unsicher) differenziert wurden, unterschieden sich tendenziell im Hinblick auf den Behandlungserfolg, wobei die ambivalent gebundenen Patient(inn)en den günstigsten Behandlungserfolg zeigten (insbesondere in symptombezogenen und auf das Selbstsystem gerichteten Outcomekriterien). Darüber hinaus ergaben sich Hinweise auf eine unterschiedliche Bewertung gruppenbezogener Wirkfaktoren durch die Subgruppen. Vermeidend gebundene Patient(inn)en scheinen subjektiv gruppenbezogene Wirkfaktoren der Behandlung, wie Kohäsion oder Altruismus, am wenigsten zu schätzen. Die Studie wird eingeordnet in die bisher vorliegenden Befunde zur Bedeutung bindungsbezogener Variablen für die Indikation und Prognose von Psychotherapien.

Abstract

Within a reanalysis of the „Kiel Group Psychotherapy Study”, the videorecorded intake interviews of 34 patients were analyzed using the adult attachment prototype rating. Three subgroups, differentiated according to their attachment patterns (avoidant, ambivalent, mixed insecure) differed marginally with respect to their treatment success, with ambivalent patients showing the most positive outcome (escpecially with regard to symptom- and self-system-related outcome criteria). In addition, there were indications for a differential assessment of group related therapeutic factors by the subgroups. Patients with an avoidant attachment pattern evaluated group-related factors such as cohesion and altruism as least helpful. The study is discussed in relation to existing findings on the significance of attachment related variables for the indication and prognosis of psychotherapy.
