Gastro-ösophageale Refluxkrankheit (GERD)

Seit etwa 10 Jahren sind endoskopische Antirefluxverfahren in der Entwicklung, zu denen jetzt erste Ergebnisse klinischer Studien vorliegen. Hierzu zählen die Injektionsmethode, bei der gelatinöse Plexiglasmikrosphären am unteren Ösophagussphinkter submukös injiziert werden [8], die Radiofrequenzinduktion, bei der die Radiofrequenzenergie über eine Spezialsonde am gastro-ösophagealen Übergang appliziert wird [27] und die Endocinch-Methode, mit der durch eine endoskopische Naht eine Gastroplicatio geschaffen wird [25]. Diese endoskopischen Therapieverfahren stellen einen interessanten Ansatz für ein ausgewähltes Patientengut dar. Insbesondere leichtere Formen der Refluxkrankheit ohne größere Hiatushernie scheinen entsprechend der Kurzzeitresultate erfolgreich therapierbar zu sein, wobei die Radiofrequenzinduktion bessere Ergebnisse aber auch höhere Komplikationsraten aufweist. Prospektiv randomisierte Vergleichsstudien sind dieses Jahr initiiert worden; deren Langzeitresultate bleiben abzuwarten. Daher sollten diese Therapien zurzeit nur innerhalb von Studien durchgeführt werden.

Die Arbeitsgruppe von Carlsen und Franzides konnte in einer umfassenden Recherche der englischsprachigen Fachliteratur mit einer Fallzahl von 10 735 Patienten aufzeigen, dass die Mortalität der laparoskopischen Antirefluxchirurgie bei 0,08 % liegt [5]. Die Konversionsrate betrug 3,7 % mit deutlich abnehmendem Trend. Als Frühkomplikation war die Manschettenherniation mit 1,3 % das häufigste Problem. Sowohl in dieser Studie als auch in einer Arbeit von Lafullarde [19] konnte in 90 % der Fälle nach laparoskopischer Antirefluxchirurgie ein gutes bis sehr gutes funktionelles Ergebnis erzielt werden.

In einer prospektiv randomisierten Studie verglich Lundell die medikamentöse Langzeitprophylaxe von 40 mg Omeprazol/die mit der Antirefluxchirurgie durch Fundoplicatio oder Hemifundoplicatio [21]. In der Intention-to-treat-Analyse zeigte der chirurgische Therapiearm (n=122) hinsichtlich des Therapieversagens hochsignifikant bessere Ergebnisse als die Vergleichsgruppe mit Einnahme einer festen Medikamentendosis (n=133). Nur bei Dosisanpassung bis 60 mg Omeprazol/die wurde dieser Unterschied wieder aufgehoben. Die Lundell-Studie demonstriert somit die Überlegenheit der Antirefluxchirurgie gegenüber der standardisierten medikamentösen Langzeitprophylaxe bei der gastro-ösophagealen Refluxkrankheit.
