Zusammenfassung
Der Gastrointestinaltrakt ist ein Reservoir für die fakultativ pathogenen Sprosspilze, insbesondere für Candida albicans. In allen seinen Abschnitten kann es bei abwehrgeschwächten Patienten zu einer Schleimhautmykose und davon ausgehend auch zu einem Befall innerer Organe kommen. Bei hämatologisch-onkologischen Patienten ist der Gastrointestinaltrakt bei 88 % aller Pilzinfektionen pathologisch-anatomisch beteiligt - deutlich öfter also als bei anderen Krankheitsgruppen (17-36 %). Am häufigsten betroffen ist der Ösophagus. Ein Befall der Schleimhaut des Magens und des Duodenums ist erheblich seltener. Die Kandidose des Darms tritt in Form einer pseudomembranösen, hämorrhagisch-nekrotisierenden oder ulzerös-abszedierenden Enterokolitis auf. Bezüglich der Diagnostik ist die Synopsis von Klinik (einschließlich endoskopischem Bild), Pilzkultur und Mykoserologie wichtig. Ein Zusammenhang zwischen dem Vorkommen von Candida im Gastrointestinaltrakt und zahlreichen „Befindlichkeitsstörungen”, wie zum Beispiel Ermüdbarkeit, Blähungen, Kopfschmerzen oder Herzbrennen, besteht nicht.
Summary
The gastrointestinal tract is a reservoir for facultative pathogenic yeasts, especially Candida albicans. In all of its sections in immunocompromised hosts the occurrence of a mucosal mycosis is possible which may be the starting point of an infection of internal organs. Pathological findings confirm a participation of the gastrointestinal tract in 88 % of all mycotic infections in haematologic-oncologic patients and therefore more frequent than in cases of other illnesses (17- 36 %). The oesophagus is the most often affected location. The affection of the stomach and duodenum is more rarely. An intestinal candidosis occurs as a pseudomembranous, haemorrhagic-necrotizing or ulcerative-abscessing enterocolitis. A synopsis of clinical (including endoscopic) findings, mycological cultivation and mycoserology is important in diagnostics. There is no connection between the incidence of Candida in the gastrointestinal tract and multiple local symptoms like fatigue, headache, heartburn and others.
