Olanzapin in der Behandlung der manischen Episode einer bipolaren affektiven Störung

Zusammenfassung

In der Behandlung der bipolaren affektiven Störung stehen neben Lithium immer mehr potenziell stimmungsstabilisierende Medikamente zur Verfügung. Die Verordnungshäufigkeit von Valproat scheint aufgrund günstigen Wirkprofils und überlegener Sicherheit gegenüber Lithium deutlich zuzunehmen. Obgleich in verschiedenen Studien nachgewiesen werden konnte, dass atypische (neuere) Antipsychotika in der Behandlung akut manischer Episoden sowohl konventionellen Neuroleptika als auch einer Monotherapie durch Lithium oder Valproat überlegen sind, ist Olanzapin das einzige atypische Antipsychotikum, welches bisher die europäische Zulassung zur Behandlung der Manie erhalten hat.

Unter Berücksichtigung der aktuellen Datenlage zeigte Olanzapin nicht nur eine deutliche Überlegenheit in der Behandlung manischer Episoden, auch hinsichtlich möglicher bzw. sinnvoller Kombinationstherapien mit Stimmungsstabilisierern und in der Langzeitbehandlung scheinen sich eindeutige Vorteile gegenüber anderen atypischen (neueren) Antipsychotika anzudeuten.

Abstract

Besides lithium ever more potential mood stabilisers are available for the treatment of bipolar affective disorders. The frequency of valproate prescriptions has increased due to its favorable mode of action and its superior safety compared to lithium. Although a number of studies have proven that in the treatment of acute manic episodes, atypical (novel) antipsychotics outperform traditional neuroleptics as well as a monotherapy with either lithium or valproate, olanzapine is the only atypical antipsychotic which has been given European approval for the treatment of mania. Recent data show that olanzapin is not only more effective in the treatment of manic episodes, but also that in combination with mood stabilisers and in long-term treatment there are significant advantages compared to other novel antipsychotics.
