Zusammenfassung
Die im Jahr 2002 verabschiedete 8. Novelle der ÄAppO macht ein zweiwöchiges Allgemeinarztpraktikum
zum verpflichtenden Bestandteil des Medizinstudiums, während es den medizinischen
Fakultäten noch weitgehend an den inhaltlichen und formalen Rahmenbedingungen fehlt,
um Praktika in Hausarztpraxen für ihre Studierenden durchzuführen.
An der Universität Witten/Herdecke wird seit Sommersemester 2000 ein Allgemeinarztprogramm
durchgeführt, das im ersten Semester beginnt und mit insgesamt sechs 14-tägigen Praxisblöcken
in ausgewählten Lehrpraxen in den gesamten Studienverlauf integriert ist. Für jeden
Praxisblock wurden eigene Aufgabenstellungen entwickelt, die sich am jeweiligen Ausbildungsstand
der Studierenden orientieren und den Ausbildungsmodulen »Wahrnehmung/psychosoziale
Kompetenz«, »Diagnostik/Anamneseerhebung«, »Therapie/Langzeitbetreuung«, »Gesundheitsberatung/Prävention«
und »Praxiskontext/Gesundheitswesen« zugeordnet sind.
Erste Evaluationen spiegeln die positive Resonanz wider, auf die das Ausbildungsprogramm
bei beteiligten Ärzten und Studierenden stößt. Patienten schätzen die mit der Ausbildung
verbundene intensivere Zuwendung und Transparenz. Es bedarf einer gezielten Vorbereitung
und Begleitung der allgemeinmedizinischen Lehrärzte wie auch der Studierenden sowie
einer inhaltlichen Strukturierung der Praktika durch Ausbildungsmanuale, die lernzielorientierte
Aufgabenstellungen festlegen und einen Transfer zu den übrigen Ausbildungsinhalten
ermöglichen.
Summary
Rotations in primary care as an approach of preparing students for a health care system
in transition
The 8th novel of the federal guidelines for medical education from 2002 calls for
a mandatory two week practicum in a general practitioner's office, whereas medical
faculties are still lacking concepts and infrastructure to implement rotations in
doctors' offices for their students. The authors of this article recommend to focus
rotations on the specifics of family medicine such as dealing with unspecified complaints,
early detection of diseases and information on health behavior, continuous patient
contact within his/her social environment and management of diagnostic and therapeutic
services. This calls for a focused preparation of students as well as participating
doctors and structured, goal-oriented manuals for the rotation.
In summer 2000, the Witten/Herdecke University started a new curriculum in general
practice. Since then it has six 14-day-rotations integrated into the entire medical
curriculum. Students choose their teaching doctor from a list of qualified cooperating
general practitioners. Each of the six rotations consists of elaborated teaching manuals
addressing specific tasks to be solved. They refer to students' level of knowledge
and are organized under the topics «perception/communication skills«, »diagnostics/history
taking«, »therapy/long-term care«, »health promotion/prevention«, and »structural
context/health system«.
Preliminary evaluations reflect the positive resonance this course has evoked among
students as well as participating doctors, while patients appreciate the increased
attention they receive and transparency of the doctor's doing.
Preparation and training of participating general practitioners as well as students
is crucial. Recommended are manuals for each rotation, listing tasks to be accomplished
and referring to overall learning goals.
