Zusammenfassung
Erhalten Frauen die Diagnose Mammakarzinom, ist dies für sie nicht nur eine medizinische
Diagnose, sie empfinden dies auch als Lebensbedrohung und fürchten eine Veränderung
des individuellen Körperbildes. Die operative Therapie beim primären Mammakarzinom
basiert auf einem interdisziplinären Stufenschema und der Festlegung des individuellen
Therapie-ablaufes. Ziel der operativen Maßnahmen ist es, eine lokale Erkrankungsfreiheit
und eine Erhaltung bzw. Wiederherstellung des körperlichen Selbstbildes der Frau zu
erreichen. Standardverfahren ist daher die brusterhaltende Therapie. Ist eine Ablatio
mammae nötig, kann im Anschluss daran eine primäre oder sekundäre Rekonstruktion erfolgen.
Grundsätzlich steht hierbei die Wiederherstellung der Brust durch alloplastisches
Material oder durch Eigengewebe zur Wahl. Neben der Lymphonodektomie als Standard
der operativen Therapie der Axilla, wird die Sentinelnodebiopsie bei bestimmten Konstellationen
zur Vermeidung der Morbidität durchgeführt.
Summary
Operative treatment of primary breast cancer is based on a interdisciplinary decision
and proposition of the therapy schedule. The diagnosis of the disease breast cancer
leads to psychological distress and variation of the individual figure. The aim of
the operative treatment leads towards local cure and reconstruction the individual
figure. The most used technic is breast conserving therapy. Breast ablation may be
followed by primary or secondary reconstruction. For reconstruction alloplastic material
or body tissue may be used. In addition to the treatment of the breast also surgery
of the axilla is done. Complete axillary clearing is the standard procedure. For defined
low risk constellation sentinel lymph node biopsy may be the choice to reduce postoperative
morbidity.
