Zusammenfassung

Das posturale Tachykardiesyndrom wurde erstmals von Da Costa vor über 100 Jahren beschrieben. Dieses Syndrom wird derzeit in der Literatur als eine heterogene Gruppe mit ähnlichen klinischen Symptomen beschrieben. Allen Patienten gemeinsam ist ein exzessiver Herzfrequenzanstieg nach Lagewechsel vom Liegen zum Sitzen und/oder Stehen, mit nur geringem oder keinem Blutdruckabfall. Fälle von Blutdruckabfall und Synkopen sind beschrieben. Über die Prävalenz und Inzidenz des POTS ist wenig bekannt. Man geht davon aus, dass bei Patienten mit Symptomen eines POTS bis vor einigen Jahren häufig eine psychosomatische Störung diagnostiziert wurde. Erst in den letzten Jahren wurden durch die Verbreitung der Kipptischuntersuchung zunehmend orthostatische Intoleranzsyndrome erkannt. Wir haben in unserem Kipptischlabor in einem Zeitraum von 18 Monaten 115 Patienten mit der Zuweisungsdiagnose „unklare Synkopen” untersucht. Die Kriterien eines POTS wurden bei 15 Patienten erfüllt. Prinzipiell werden eine primäre Form mit zwei Subtypen und eine sekundäre Form unterschieden. Die Diagnose erfordert eine ausführliche Anamnese und im Weiteren eine Kipptischuntersuchung. Therapeutisch werden sowohl konservative als auch medikamentöse Maßnahmen eingesetzt. Über die Lebenserwartung dieser Patienten gibt es derzeit in der Literatur keine Angaben. Patienten mit einem POTS haben jedoch eine deutlich eingeschränkte Lebensqualität, vor allem im physischen und sozialen Bereich. Dies unterstreicht die Notwendigkeit einer ausführlichen Anamnese und Diagnostik bei „scheinbar” unauffälligen Patienten mit orthostatischen Beschwerden.

Abstract

The postural tachycardia syndrome was firstly described by Da Costa 100 years ago. It is characterized by an excessive rise of standing heart rate without postural blood pressure drop. Cases with syncope were described. There are no clear data about the incidence and prevalence of POTS. Until a few years ago many patients with POTS have been diagnosed as a psychosomatic disorder. Examination with the tilt table gave the possibility to recognize more orthostatic intolerance syndromes including the POTS. In our autonomic laboratory 115 patients with unclear syncope have been examined so far. 15 patients fulfilled the criteria of POTS. In the literature a primary form of POTS with two subtypes and a secondary form have been described. Its aetiology is poorly understood, however, if treated appropriately, this often results in improvement of otherwise markedly impaired quality of life. The diagnosis includes detailed history and further a tilt table testing. The therapeutic approach includes physical training and medication as well. Recognition of POTS is essential in the differential diagnosis of orthostatic dysregulation and the clinical suspicion of POTS can be substantiated by tilt table testing in most instances.
