Cystic Lymphangioma of Retzius Space Manifested as Acute Abdomen

Abstract

The authors describe a rare pediatric case of cystic lymphangioma arising from the Retzius space. A 9-year-old boy underwent an appendectomy in a nearby hospital after a sudden onset of severe hypogastralgia. When laparotomy revealed a retroperitoneal mass, he was referred to our hospital. After diagnosis of a multicystic mass in the Retzius space, extirpation of the cystic lesion was performed. Histological evaluation of the resected specimens revealed cystic lymphangioma. The patient has been free of symptoms for 6 years since the operation.

Les auteurs décrivent un cas rare de lymphangiome kystique développé dans l'espace de Retzius. Un garçon de 9 ans a subi une appendicectomie pour des douleurs de la région hypogastrique. La laparotomie révélait une masse rétro-péritonéale qui a entraîné le transfert de l'enfant dans notre hôpital. Le diagnostic d'une masse multi-kystique de l'espace de Retzius a été posé et l'exérèse de la masse réalisée. L'étude histologique a révélé qu'il s'agissait d'un lymphangiome kystique. L'enfant va bien, 6 ans après l'opération.

Los autores describen un raro caso de linfangioma quístico que apareció en el espacio de Retzius. A los 9 años un paciente varón fue operado de apendicitis por un cuadro de comienzo brusco de dolor hipogástrico en un hospital cercano. Durante la laparotomía se observó una masa retroperitoneal y nos fue remitido. Tras el diagnóstico de masa multiquística en el espacio de Retzius se realizó la extirpación. Los hallazgos histológicos revelaron un linfangioma quístico. El paciente ha permanecido asintomático durante los 6 años que han pasado desde la intervención.

Zusammenfassung

Die Autoren beschreiben den seltenen Fall eines zystischen Lymphangioms, das vom Spatius Retzii ausging. Bei einem 9-jährigen Knaben wurde eine Appendektomie durchgeführt, nachdem er plötzlich über starke Schmerzen im Bereich des rechten Unterbauches klagte. Bei der Laparotomie fand sich ein retroperitonealer Tumor, weshalb das Kind in die Klinik überwiesen wurde. Es wurde ein CT durchgeführt sowie ein i. v. P. Beide Untersuchungen zeigten die zystischen Veränderungen im Sinne eines multizystischen, dem Spatium Retzii entspringenden Lymphangioms. Der Tumor konnte vollständig und ohne Rezidiv in einem Beobachtungszeitraum von 6 Jahren entfernt werden. Der Fall ist aufgrund seiner fehlerhaften Differenzialdiagnose interessant und weist darauf hin, dass grundsätzlich bei Verdacht auf Appendizitis eine Ultraschalluntersuchung heute obligatorisch sein muss. Darüber hinaus sind Lymphangiome, welche dem Spatium Retzii entspringen, außerordentlich selten.
