Zusammenfassung

Das Stromasarkom der Mamma weist Lokalrezidive und auch Fernmetastasen auf, ist insgesamt selten und geht mit schlechter Prognose einher. Die Kasuistik zeigt einen komplexen Verlauf dieser Erkrankung ausgehend von einem ausgedehnten Initialbefund. Die angefertigte Bildgebung legte eine Infiltration ins Mediastinum nahe. Im Verlauf erfolgte die chirurgische Sanierung des Lokalbefundes und dessen Rezidiven. Überraschenderweise demaskierte sich die mediastinale Raumforderung aber nicht als Metastase, sondern als vom Mammasarkom unabhängiger Zweittumor in Form eines Thymuskarzinoms. Der Fall zeigt, dass trotz eindeutiger Klinik differenzialdiagnostische Überlegungen anzustellen sind und dass durch die Kooperation verschiedener Kliniken auch bei komplexen Verläufen Erfolge erzielt werden können.

Abstract

Stromal breast sarcomas have a high probability of local recurrency and frequent metastasis. It is a rare disease with a bad prognosis. The case presented here shows a complex process starting with an extensive tumor. Computer tomography suggested an infiltration of the mediastinum. Following several surgical interventions to treat the primary tumor, the mediastinal finding surprisingly revealed an independent second carcinoma of the thymus gland. In cases in which clinical impressions seem to be clear, differential-diagnostic considerations are nevertheless necessary to deal with complicated diseases. The case shows that treatment can be successful when different departments cooperate, even in complex epicrises like this.
