Abstract
Aim: The aim of this report was to assess the effects of radiotherapy (RT) in children
with abdominopelvic tumours in whom a biodegradable (Polyglactin 910) mesh had been
inserted prior to commencement of radiotherapy. Methods: Retrospective analysis was carried out of four patients with abdominopelvic tumours
who underwent radiotherapy between 2000 and 2002 as part of their management. Results: All children tolerated radiotherapy well with no evidence of acute or chronic radiation
enteritis. One child developed prolonged postoperative ileus and a second child developed
infective diarrhoea and fever, not related to radiation. Conclusion: We have highlighted a good tolerance of radiotherapy in children following the insertion
of a Polyglactin 910 mesh prior to starting radiation and would recommend further
larger studies with longer follow-up to support this.
Résumé
Buts: Le but de cette étude est d'apprécier les conséquences de la radiothérapie (RT) chez
l'enfant traité pour des tumeurs abdomino-pelviennes pour lesquelles une plaque biodégradable
(polyglactine 910) a été insérée avant le début de la radiothérapie. Méthodes: L'analyse rétrospective de quatre patients présentant une tumeur abdomino-pelvienne
ayant subi une radiothérapie entre 2000 et 2002 comme partie de leur traitement. Résultats: Tous les enfants ont bien tolérés la radiothérapie sans entérite aiguë ou chronique.
Un enfant a développé un iléus post-opératoire prolongé et un second a développé une
diarrhée avec de la fièvre sans lien avec l'irradiation. Conclusion: Nous avons mis en évidence la bonne tolérance de la radiothérapie chez les enfants
après insertion d'une lame de polyglactine 910 avant de commencer l'irradiation et
nous voulions recommander des études plus larges avec un plus long suivi pour confirmer
ceci.
Resumen
Objetivo: Este estudio investiga los efectos de la radioterapia (RT) en niños con tumores abdominopélvicos
en quienes se colocó una malla biodegradable (Polyglactin 910) antes de comenzar el
tratamiento. Métodos: Análisis retrospectivo de 4 pacientes con tumores abdominopélvicos que fueron tratados
con radioterapia entre los años 2000 y 2002 como parte de sus protocolos. Resultados: Todos los niños toleraron bien la radioterapia sin evidencia de enteritis por radiación
aguda o crónica. Uno desarrolló un ileo postoperatorio prolongado y otro una diarrea
infecciosa con fiebre no relacionada con la radiación. Conclusión: Hemos demostrado la buena tolerancia a la radioterapia en niños tras la inserción
de una malla de Polyglactin 910 antes de iniciarla y recomendamos que se realicen
estudios más amplios con seguimientos más largos para verificar esto.
Zusammenfassung
Zielsetzung: Ziel der vorliegenden Arbeit war es, die Wirksamkeit der Radiotherapie bei vier Kindern
mit einem abdomino-pelvinen Tumor, bei denen ein „biologisches“ Netz (Polyglactin
910) vor dem Beginn der Radiotherapie implantiert wurde, einzuschätzen. Methodik: Eine retrospektive Analyse wurde bei diesen vier Kindern durchgeführt. Diese Patienten
wurden zwischen 2000 und 2002 einer Radiotherapie als Teil ihrer Behandlung unterzogen.
Ergebnisse: Alle Kinder tolerierten die Radiotherapie gut, ohne Zeichen einer akuten oder chronischen
Bestrahlungsenteritis. Ein Kind entwickelte einen verzögerten postoperativen Ileus,
ein zweites Kind eine infektiöse Diarrhö mit Fieber, die jedoch nicht auf die Bestrahlung
zurückgeführt werden konnte. Schlussfolgerung: Diese vorläufigen Ergebnisse zeigen, dass die Implantation eines Polyglactin-910-Netzes
vor der Bestrahlung offensichtlich das sonst hohe Risiko einer Strahlenenteritis zu
reduzieren vermag. Allerdings müssen diese Ergebnisse durch eine ausgedehntere Studie
noch bestätigt werden.
