Zusammenfassung

Hintergrund: Im Falle des CUP-Syndroms kann die Positronen-Emissions-Tomographie (PET) wertvolle Zusatzinformationen liefern. Die Fusion von PET und Computer-Tomographie im integrierten PET/CT erleichtert die anatomische Zuordnung der hypermetabolen FDG-Herde im CT. Die fusionierte Bildgebung scheint auch beim CUP-Syndrom Fortschritte in der Lokalisation kleiner und auch submukös gelegener Tumoren zu bedeuten. In der vorliegenden retrospektiven Studie soll an einem größeren Patienten-Kollektiv untersucht werden, in welchen Fällen die PET bzw. das PET/CT einen Vorteil in der Primärtumor-Suche darstellen können. Patienten: An einem nicht selektiven Patientengut wurden 49 PET- und PET/CT-Befunde von 47 Patienten mit Hals-Lymphknoten-Metastasen bei unbekanntem Primärtumor retrospektiv untersucht. Ergebnisse: In 8 von 47 Fällen konnte der Primärtumor durch das PET detektiert werden, 1 Fall war falsch positiv, 5 Fälle waren falsch-negativ. Schlussfolgerung: Die Rate der entdeckten Primärtumoren kann durch PET und PET/CT gesteigert werden. Da es Hinweise gibt, dass sich Prognose und Überlebensrate der Patienten durch die Feststellung und gezielte Behandlung des Primärtumors verbessern lassen, sollte die PET in entsprechenden Zentren in die Diagnostik bei CUP einbezogen werden.

Abstract

Background: In the last years 18F-FDG-positron-emission-tomography (PET) worked satisfactorily as helpful auxiliary method in order to verify recurrency of head and neck tumors and to detect primary tumors in case of CUP syndrome especially when CT and MR imaging failed to identify the tumor accurately. Fusion of FDG hypermetabolism in PET scan and anatomical structures is achieved by integrating positron emission tomography with CT and provides improvement also in case of CUP syndrome. This retrospective study shows 47 patients with neck metastases where PET or PET/CT helped to detect primary tumor site. Patients: In a retrospective investigation 49 PET studies of 47 patients with CUP syndrome were analyzed. Results: 9 cases had positive PET findings, 1 case false-positive. 5 cases were false-negative. In 40 PET studies there couldn't be found any sign of suspicious FDG hypermetabolism. Conclusion: PET and PET/CT deliver a certain improvement in localization of primary tumor site and therapeutical strategy.
