Zusammenfassung
Mit Dronedaron wird in absehbarer Zeit ein neues Antiarrhythmikum zur Verfügung stehen.
Die Phase III der klinischen Entwicklung ist abgeschlossen und der Antrag auf Zulassung
bei den europäischen und US-amerikanischen Arzneimittelbehörden eingereicht. Bei Dronedaron
handelt es sich um eine Weiterentwicklung von Amiodaron. Die wesentlichen Unterschiede
sind das Fehlen der Jodkomponente sowie die schwächere Lipophilie und kürzere Halbwertszeit.
Elektrophysiologisch sind die Charakteristika aller vier Vaugham-Willams-Klassen vorhanden.
Bei Vorhofflimmern hat sich Dronedaron sowohl zur Rhythmuskontrolle als auch zur Frequenzkorrektur
als effektiv erwiesen. Die Verträglichkeit entsprach in etwa der von Plazebo. Anhaltspunkte
für ein proarrhythmisches Potential oder (Amiodaron-typische) extrakardiale Nebenwirkungen
gibt es bisher nicht. Unklar ist derzeit noch der Stellenwert von Dronedaron bei Patienten
mit fortgeschrittener Herzinsuffizienz oder eingeschränkter Nierenfunktion.
Summary
A new anti-arrhythmic dug, dorenadarone, will become available in the foreseeable
future. Phase III of its clinical evaluation has been concluded and approval by the
European and US drug regulatory authorities has been applied for. Dronedarone is further
development of amiodarone, the main differences between these two drugs being that
the former contains no iodine, weaker lipophilia and has a shorter half-life. It has
the electrophysiological properties of properties of all four classes after Vaugham-Williams.
Dronedarone has been shown to be efficacious in both rhythm and ventricular rate control.
It is tolerated about as well as the placebo. There is so far no evidence of a pro-arrhythmic
potential or (amiodarone-typical) extracardiac side-effects. The place of dronedarone
in patients with advance heart failure or impaired renal function is not yet clear.
