Aim: The purpose of this project is to illustrate, through hand-drawn figures, the technique of the postauricular and endaural mastoidectomy combined with tympanoplasty.

Method: We present the following chapters: tympanoplasty with mastoidectomy; facial nerve decompression in chronic otitis media; revealing of the jugular bulb; simple mastoidectomy techniques; opening of the facial recess; cholesteatoma topography in the mastoid cavity; landmarks for identification of mastoid and tympanic segments of the facial nerve in chronic otitis media; facial nerve in chronic otitis media; tympanoplasty; incus remnant removal; myringoplasty: techniques for temporalis fascia graft placement in tympanic membrane perforation; tympanoplasty—mastoidectomy (canal wall down technique or open cavity); functional restoration of hearing; mastoid cavity obliteration techniques; meatoplasty techniques.

Results and Conclusions: We believe that this manual describing alternative techniques of surgical steps, movements, and instruments is useful for a thorough didactical update on better treatment in cases of chronic otitis media.
