Prophylaktische Maßnahmen vor einer schwierigen Intubation

Schwierige Intubationen kommen immer wieder vor. Leider werden sie zum voraus oft nicht erkannt. Dieser Artikel weist auf die wichtigsten prophylaktischen Maßnahmen hin, um den damit verbundenen Gefahren und Risiken vorzubeugen. Dazu gehören: die Untersuchung (Morphologie, Beweglichkeit, Laryngoskopie), die Lagerung des Patienten, die Aspirationsprophylaxe, die Wahl und die Vorbereitung des Materials sowie der Medikamente, die nochmalige Frage nach der Notwendigkeit der Intubation, die Oxygenierung sowie die Kontrolle der Beatmungsmöglichkeit vor Muskelrelaxation. Besonders die beiden letzterwähnten Maßnahmen dürften in der Mehrheit der Fälle die wichtigsten sein.

Summary

Difficult intubations occur frequently. Unfortunately they cannot always be recognized before intubating. This paper describes the most important prophylactical measures which can prevent risks during intubation. These are: examination (morphology, mobilization of the joints, laryngoscopy), optimal positioning of the patient, aspiration prophylaxis, choice and preparation of equipment and drugs, the repeated question of necessity of intubation, the oxygen supply and the attempt to ventilate the patient before giving muscle relaxants. Particularly the last two measures are of great importance in the majority of cases.
