Zusammenfassung

Die Karotis-Dopplersonographie stellt heute eine anerkannte Methode zur Erfassung obstruktiver Prozesse im Bereich der Art. carotis interna dar. Die Beschallung der Art. supratrochlearis und der Aa. carotis communis, interna und externa im Halsbereich ist eine Untersuchung, mit deren Hilfe Lokalisation der Stenose und Pathophysiologie des Kollateralkreislaufes erfaßt werden können. Die Treffsicherheit dieser Methode liegt bei unseren Untersuchungen bei Stenose über 50% und Verschlüssen der A. carotis interna bei 92%. In 8% der Fälle wurde ein falsch negativer und in 10,1 % ein falsch-positiver Befund erhoben. Die falsch-negativen Befunde resultierten bei Carotis interna-Stenosen um 50% Lumeneinengung (7 Fälle), bei extremer Hypertonie (3 Fälle), bei kontralateralem Carotis interna-Verschluß (3 Fälle) und kontralateralem Carotis interna-Verschluß und ipsilateralen Mediastenosen (2 Fälle). Falsch positive Befunde wurden bei A. ophathalmica-Stenosen (3 Fälle), bei Aortenvitien (4 Fälle), Malazieim Mediagebiet (1 Fall) sowie in 11 Fällen ohne eine Ursache dafür zu finden, erhoben.

Abstract

Today Doppler ultrasound examination is recognized as a very valuable method for detecting obstructive lesions of the internal carotid artery. Percutaneous measurements by Doppler techniques over the supratrochlear, common carotid and internal carotid arteries in the neck can detact the presence of a stenosis in the internal carotid artery. It is also possible to locate accurately the site of the Stenosis in the neck and to verify the pathophysiology of the collateral circulation. Out of 1900 patients examined by Doppler ultrasound, 241 patients underwent carotid angiography, and the angiographic data were compared with the non-invasive results. The diagnostic accuracy was 92% in cases of Stenosis amounting to more than 50%, and with occlusions of the internal carotid artery. Doppler examination yielded 8% false negative and 10.1 % false positive results.

False negative results were obtained in patients with stenoses of around 50% narrowing of lumen (7 patients), in extreme hypertension (3 patients), in contralateral occlusion of the internal carotid artery (3 patients) and in patients with contralateral occlusion of the internal carotid artery and homolateral Stenosis of the median cerebral artery (2 patients). False positive results were found in patients with Ophthalmic artery Stenosis (3 patients), in patients with aortic Stenosis (4 patients), and in one case of ischemic infaretion in the median cerebral artery area. In 11 cases we could not find any cause for the false positive results.
