Zusammenfassung

Tumoren im linken Vorhof, vorwiegend Myxome, sind entweder mobile, gestielte, diastolisch in die Mitralklappen prolabierende oder seltener wandständige, unbewegliche Gebilde, welche in das Kavum des linken Vorhofes hineinwachsen. Bei den mobilen Tumoren erlaubt schon die M-mode-Echokardiographie in den meisten Fällen eine sichere Diagnosestellung, der Sektor-Scanner ermöglicht zusätzlich die Bestimmung der Größe und genaue Lage des Tumors. Bei den echokardiographisch nicht so typischen unbeweglichen Tumoren im linken Vorhof ist die Differentialdiagnose wesentlich schwieriger, da auch mit dem Sektor-Scanner in einem Teil der Fälle keine sichere Abgrenzung gegenüber Thromben im linken Vorhof oder der Kompression des Vorhofes durch mediastinale Prozesse möglich ist. Die klinisch sehr schwierige Diagnose eines Vorhoftumors wurde durch die Echokardiographie wesentlich erleichtert, so daß heutzutage im größeren Teil der Fälle auf eine präoperative invasive Abklärung verzichtet werden kann.

Abstract

Tumors in the left atrium, usually myxomas, are either mobile, pedunculated growths prolapsing diastolically into the mitral valves, or, more rarely, marginal, immobile structures which grow into the cavity of the left atrium. In the case of the mobile tumors, M-mode echocardiography will mostly enable safe diagnosis, the sector Scanner additionally providing information on the size and exact site of the tumor. Differential diagnosis is much more difficult with the immobile tumors in the left atrium, which are echocardiographically not so typical. In some cases, no safe delimitation is possible against thrombi in the left atrium or against compression against the atrium by mediastinal processes, even with the sector Scanner. The clinically very difficult diagnosis of a tumor in the atrium is significantly facilitated by echocardiography, so that in the majority of cases it is now possible to abstain from preoperative invasive examination.
