Protein C Response to Induction of Warfarin Treatment after Coronary Bypass Operation

Summary

To investigate the effect of warfarin on the anticoagulant pathway, protein C antigen and activity levels were compared in two groups of patients treated with different regimens of sodium warfarin during the initial stage of anticoagulant therapy following heart surgery. Group I received 6 mg of warfarin per day for 3 days. Group II received 8 mg twice a day for 2 or 3 days. Preoperative levels of Protein C antigen and activity averaged 108 ± 16 % and 102 ± 18 % (mean ± SD), respectively, and by one hour after the operation, levels had fallen significantly (protein C antigen to 76 ± 14 %; protein C activity to 70 ± 16 %). After the initiation of warfarin treatment, the levels of protein C activity in group II were significantly lower than those of group I. In contrast, the reductions in factor X were much slower and were similar in the two groups. As the factor X level reflects the antithrombotic effect of warfarin, this result suggests that the rapid reduction of protein C activity may have given rise to a transient hypercoagulable state in the patients of group II.

Zusammenfassung

Um den Einfluß von zwei verschiedenen Warfarin-Einstiegsdosierungen auf die Antikoagulation nach aortokoronarer Bypassoperation zu untersuchen, wurde sowohl die Menge als auch die Aktivität des Protein C in der initialen Phase bestimmt. Dazu wurden 2 Gruppen mit je 8 Patienten verglichen, die 40 Stunden nach der Operation wie folgt behandelt wurden: Gruppe I erhielt 6 mg Warfarin pro Tag über drei Tage, Gruppe II dagegen 8 mg zweimal täglich für zwei oder drei Tage. Die präoperativen Spiegel der Protein C Aktivität und des Protein C Antigens fielen eine Stunde nach der Operation signifikant ab (102 ± 18 % vs. 70 ± 16 % und 108 ±16 % vs. 76 ± 14 %). Nach Beginn der Warfarintherapie zeigte sich in Gruppe II ein signifikanter Abfall der Protein C Aktivität im Vergleich zu Gruppe I. Im Gegensatz dazu war der Abfall des Faktors X wesentlich langsamer, ohne signifikante Unterschiede zwischen beiden Gruppen. Da der Faktor X Spiegel jedoch ein Maß für die antithrombotische Wirkung des Warfarins darstellt, legen unsere Ergebnisse die Vermutung nahe, daß durch den deutlich schnelleren Abfall der Protein C Aktivität in Gruppe II sich hier vorübergehend ein Übergewicht hin zu einem hyperkoagulabilen Zustand bildet. Aus diesem Grund befürworten wir eine niedrige Anfangsdosierung des Warfarins, in Kombination mit einer plättchenhemmenden und antikoagulierenden Therapie in der unmittelbar postoperativen Phase, da ausreichende therapeutische Warfarin-Spiegel in der Regel erst am fünften postoperativen Tag zu erreichen sind, und der Beginn einer Warfarintherapie zusätzlich durch einen nur unzureichenden antithrombotischen Schutz aufgrund signifikant erniedrigter Protein C Spiegel gekennzeichnet ist.
