Summary
Since the onset of the AIDS pandemic in 1981, acute and chronic inflammatory infectious diseases of the central nervous system have become an increasing problem for the clinical neurologist. Directly HIV-1-associated diseases (such as acute meningoencephalitis during seroconversion, chronic HIV encephalitis and immune vasculitis) as well as so-called “opportunistic infections” with parasites (such as toxoplasma gondii), fungi (cryptococcus neoformans, Candida albicans), typical and atypical mycobacteria and viruses (herpes family and cytomegaly virus) demand specific and sometimes invasive (e.g. stereotactic brain biopsy) diagnostic and therapeutic procedures. The increasing number of iatrogenic immunosuppressed, but also of immunocompetent patients infected with “classic” HPV-1-associated “opportunistic” disease requires extension of the clinician's diagnostic knowledge.
Zusammenfassung
Seit Beginn der AIDS-Pandemie 1981 werden akute und chronische infektiös-entzündliche ZNS-Erkrankungen zunehmend zu einem Problem für den klinisch tätigen Neurologen. Sowohl direkt HIV-assoziierte Krankheitsbilder (z.B. akute Meningoenzephalitis im Rahmen der Serokonversion, chronische HIV-Enzephalitis und -Immunvaskulitis) als auch sogenannte „opportunistische Infektionen” mit Parasiten (u.a. Toxoplasma gondii), Pilzen (u.a. Kryptokokkus neoformans, Candida albicans), atypischen und typischen Mykobakterien und Viren (u. a. Herpesfamilie, Zytomegalie) fordern spezifische, gegebenenfalls invasive Diagnostik und Therapie, z. B. stereotaktische Hirnbiopsie. Das zunehmende Übergreifen sogenannter „klassischer” HIV-assoziierter opportunistischer Keime auf iatrogen Immunsupprimierte, aber auch auf Immungesunde (!) erfordert die Erweiterung des differentialdiagnostischen klinischen Denkens.
