Zur Häufigkeit bilateraler maligner Melanome der Aderhaut

Zusammenfassung

Hintergrund: Das maligne Melanom der Aderhaut stellt die häufigste primäre bösartige intraokulare Geschwulst dar, es ist dennoch ein seltener Tumor. Das bilaterale Auftreten gilt als absolute Rarität. Schätzungen zufolge beläuft sich das individuelle Risiko auf 1:50 Millionen. Material und Methoden: Eine retrospektive Untersuchung der Fotodokumentationen von Patienten mit Aderhautmelanomen über einen Zeitraum von 1970 bis 2006 wurde durchgeführt. Ergebnisse: 4 Patienten mit dem Auftreten primärer maligner Aderhautmelanome an beiden Augen konnten ermittelt werden. Die Kasuistiken der zwei Männer und zwei Frauen werden dargestellt. Schlussfolgerungen: Vor dem Hintergrund einer zunehmenden Fallzahl publizierter bilateraler Aderhautmelanome stützen die eigenen Beobachtungen die Hypothese eines höheren Vorkommens bilateraler Aderhautmelanome als bislang angenommen. Die Wahrscheinlichkeit, bei Vorliegen eines Melanoms der Uvea im Laufe des Lebens auch am zweiten Auge an einem uvealen Melanom zu erkranken, beträgt etwa 0,2 %. Dabei kann das Intervall im Einzelfall bis zu 30 Jahre betragen. Dies impliziert lebenslange sorgfältige Nachkontrollen auch des primär nicht betroffenen Auges.

Abstract

Background: Malignant melanoma of the choroid is the most common ocular primary malignancy, but is still a rare tumour. The occurrence of bilateral uveal melanoma is exceedingly rare. The probability of any one individual developing bilateral melanoma is estimated to be 1 to 50 million. Materials and Methods: A retrospective search of the photo database of patients between 1970 and 2006 with uveal melanoma was performed. Results: Four patients with bilateral melanoma were identified. The case reports of the two females and two males are presented. Conclusions: In summary, bilateral primary uveal melanoma seems to occur more frequently than expected. The estimated probability for patients with unilateral primary uveal melanoma of developing bilateral melanoma is 0.2 %. In single cases the interval between the occurrence of the second melanoma can be more than 30 years. In patients with a history of malignant melanoma of the choroid it is important to carefully observe the other eye life-long.
