Chirurgische Therapie der deszendierenden thorakalen Aorta

Die deszendierende thorakale Aorta kann chirurgisch, konservativ-medikamentös (insbesondere bei der unkomplizierten Typ-B-Dissektion) oder interventionell (Stentimplantation) therapiert werden. Die Stentimplantation und die Operation können als Hybridverfahren - ein- oder zweizeitig - kombiniert werden.

Indikationen für die chirurgische Therapie oder die interventionelle Stentimplantation sind:

  • komplizierte Typ-B-Dissektionen,

  • Koarktationen,

  • Transsektionen und

  • Aneurysmen mit einem Durchmesser ab etwa 60 mm.

Welches Verfahren eingesetzt wird, muss individuell entschieden werden. Bei der Festlegung des Operationszeitpunktes ist zu berücksichtigen, wie schnell das Aneurysma größer wird. Insbesondere bei Patienten mit chronischer Typ-B-Dissektion und/oder Marfan-Syndrom, die ein Aneurysma ausbilden, ist häufig eine Behandlung schon ab einem geringeren Aortendurchmesser erforderlich.

Erkrankungen der thorakalen Aorta und ihre Therapien gehen trotz wesentlicher Fortschritte im perioperativen Management und im Patientenmonitoring nach wie vor mit dem Risiko einer Paraplegie oder Paraparese einher. Über dieses Risiko müssen die Patienten aufgeklärt werden.

Für Patienten mit Erkrankungen der Aorta, insbesondere bei allen Formen der Aortendissektion, ist es entscheidend, dass sie in Schwerpunktzentren behandelt werden. Dort ist eine enge Kooperation der involvierten Teilgebiete und eine individuelle und zügige Erstellung der Behandlungsstrategie gewährleistet.

In der Nachbehandlung ist es wichtig, die weiterbehandelnden niedergelassenen Kollegen einzubinden, da eine konsequente Verlaufsbeobachtung der Patienten zwingend erforderlich ist. Nur so können Verlaufskomplikationen rechtzeitig aufgedeckt werden.
